Target Case Manager (TCM) – Bachelor Level – Contract State of Florida
The TCM will work to optimize clients functioning who have complex needs by coordinating the provision of treatment and support services.
All TCMs MUST be certified by the Florida Certification Board as a Certified Behavioral Health Case Manager (CBHCM). We are in need for the following specific Florida Counties:
Hillsborough/Pinellas, & Pasco Counties
Miami/Dade & Monroe Counties
Orange/Osceola Counties
Polk County (Lakeland & surrounding area)
Qualifications include:
* Have a bachelor’s degree from an accredited university or college.
* Must be certified as a Certified Behavioral Health Case Manager (CBHCM) through the Florida Certification Board.
* Have knowledge of available resources in the service area for children with serious emotional disturbances.
* Is knowledgeable of and comply with state and federal statutes, rules and policies that affect the target population.
Requirements:
Contractors must pass Level 2 FDLE background screen (for additional information about out the clearinghouse) visit https://info.flclearinghouse.com , local background screen in county of residence, carry professional liability coverage ($1/$3M), and have the ability to learn and utilize Electronic Health Record. You must also reside in the State of FL.
Contract rate is $25.00 per billable hour.
