TCM Housing Program

Families First of Florida, LLC (FFF) is dedicated to empowering individuals with dignity and respect, supporting their unique values, beliefs, and preferences. Through our comprehensive behavioral health services, including Targeted Case Management, we strive to enhance the mental and physical well-being of homeless adults across Florida. Our Targeted Case Management Housing program is designed to assist adults in strengthening their health, securing and maintaining housing, improving personal relationships, and achieving their life goals. Families First of Florida (FFF) offers transitional housing services as part of its Targeted Case Management Housing program. These services are designed to prepare and support individuals experiencing homelessness in transitioning into permanent housing. FFF also provides tenancy sustaining services, which support individuals in becoming successful tenants and maintaining stable housing.

 

Potential clients must meet criteria for the program, which are:

  • Be 21 years of age or older
  • Be a Medicaid recipient
  • Be diagnosed with a Severe Mental Illness (SMI) (excluding PTSD and Anxiety Disorders), Substance Abuse Disorders (SUD), or co-occurring SMI with SUD
  • Be homeless or at risk of homelessness and able to live independently
  • Must live in Pasco, Pinellas, Seminole, Orange, Osceola, or Brevard County
